When ALL records are requested for a one year period (notes from all the providers and all the laboratory and diagnostic tests), the volume is amazing. It’s a deforestation project – thousands  of pages . Countless staff hours. This does not reflect the philosophy of patients over paperwork!
This is one reason the proposal to allow practitioners to document LESS rings hollow. We are just going to get audited and have our services denied for insufficient documentation.
If this is what is required for only 2 chart audits, think of the volume of paper records pouring into the MACs for all the services under audit. How many people does the MAC employ to read these records?
